Some of these anchor bolts were set to low to accept the specified washers and allow the nut to be fully threaded. Notching of the sill plate to accommodate a washer and nut is not permitted unless designed.
More importantly, note that there are two plates (the joint is under the coil of nails). There is not anchor bolt within 12 inches of the end of the plate to the right, a violation.
